Please only book this appointment if requested to do so by a GP. Please note, if you have a hospital/consultant blood test form, you will NOT be seen. Please ensure you bring a paper version of your blood test form (provided by your GP) to your appointment otherwise, you will NOT be seen. 
Please note if a patient arrives over 10 minutes late, the phlebotomist may refuse the blood test.
